The leadership sets out basic information about the prevalence and nature of the course of diabetes in elderly and senile patients, the factors contributing to its development, the characteristic of complications typical of the elderly. From the standpoint of evidence -based medicine, modern approaches to compensation for carbohydrate metabolism parameters are presented. A complete characteristic of the main groups of sugar-lowering drugs used in the treatment of type 2 diabetes mellitus, the mechanisms of their action, pharmacokinetic properties, efficiency and safety parameters, which are relevant for elderly and senile patients are reflected in detail. From the point of view of personalized medicine, the features of the dosing of sugar -lowering drugs in such patients with a different functional state of the kidneys and liver are considered, as well as against the background of the presence of other concomitant diseases.
