“The History of the Development of the Child” is the main medical document of the children's clinic, as well as children's nurseries, nursery groups of nurseries-sads, houses of the child. It is intended to continue to keep track of observation of the development and state of health of the child and about his medical care from birth to 14 years old (schoolchildren - until the end of the secondary school).

The history of the child’s development is filled in for each child when taking him on accounting: in a children's clinic - with the primary patronage (calling home) or the first appeal to the clinic; In the nursery, nurseries -sads and houses of the child - from the moment of admission to the preschool institution.

Passport part of the history of the development of the child, including Regarding the information about the composition of the family, is filled in the reception of the clinic when taking it on the basis of information from the maternity hospital, the maternity ward of the hospital about the newborn (form No. 113/y) “Exchange card” (maternity hospital, maternity ward of the hospital) or medical certificate of birth (f. No. 10z/y) and a survey of parents.

The lack of a medical birth certificate or registration data is not a basis for refusing to maintain a child.

In children's nurseries, nursery groups of nursery-sads and houses of the child, the passport part is filled with a nurse.

A nurse (in a children's clinic - precinct) also fills out the “Family Information” section when the child is first visited at home or the first appeal to the children's clinic regarding the presence of chronic diseases in the family. To register the current observations of the patronage nurse for the child, the last section of the history of the child’s development is intended.

All other records are made by doctors of all specialties in the order of current observation. All records made by doctors must be signed by them.

The history of the child’s development is stored in the registry card from birth and is transferred to the doctor on the day of the child’s visiting the clinic or visiting the child to the child at home.

The history of the development of a child for children under 1 year, as a rule, are stored in the office of the local pediatrician for the operational use of them in order to ensure systematic monitoring of the child and timely preventive measures.

When the child is displayed from the observation of this children's clinic on the title sheet of “History of the child’s development”, the appropriate mark is made: the date of deregistration and cause (move, death; disposal from the children's institution) is indicated. When moving, it is necessarily indicated - where it was left (address). In this case, in order to ensure continuity in observing the child, his “development history” on request from a new place of residence should be transferred to the relevant children's clinic. In the absence of a request “History” is stored in the registry of 3 years, and then surrendered to the archive.

When a child reaches the age of 15 years (or after graduation), “History of Development” is transferred to the clinic for adults at the place of residence.

“The history of the development of the child” is not only a medical, but also a legal document. It is not allowed to carry out cleaning, crossings, changes and additions in the records of current observations.

“The History of the Development of the Child” is used to draw up an annual “Review of a medical institution” Form N9 30 and report No. 31 “On Medical Aid to Children and Teenagers-Schools”.
